Overview - Case Manager - Supportive Housing Services

- Location: Boston, MA (Onsite, but 1 day/week is remote!)
- Position Type: Full-Time, Temporary
- Hourly Rate: $24.76
Key Responsibilities
- Develop and track Individual Service Plans (ISPs) addressing financial, health, educational, vocational, and life skills goals
- Collaborate with community partners to assist clients with housing options and necessary services
- Offer move-in transition support, including furnishings, budget assistance, and more
- Maintain detailed case records and track progress using our data management system
- Participate in team meetings, trainings, and on-call rotation
Qualifications
- Associate’s Degree with 2 years of related experience OR Bachelor’s Degree with 1 year of relevant experience
- Experience in crisis intervention, especially related to homelessness, mental health, and housing barriers
- Strong communication, time management, and organizational skills
- Ability to work independently and as part of a team
- Knowledge of community resources and case management best practices a plus
- Valid MA driver’s license and reliable transportation preferred
- Ability to travel independently between sites and necessary appointments
